Program-level Assurance Sign-Off — Documentation & Knowledge Program (G3 gate evidence)
From: Quality & Risk Director (second line of defense — independent assurance). For: the G3 gate (master plan §9 “Phase 2 / Program DoD”; tracker §1 gating checkpoint G3 “M5 → done”). This is the consolidated Q&R sign-off for program completion. It does not re-sample every artifact — it confirms the §9 Program DoD criteria are met across the whole corpus and that the conditions raised in my three per-wave samples are cleared. Basis (per R11 — the only two valid proofs): (1) the scripted reconciliation gate (
scripts/handoff-reconcile.mjswrite-outdocs/handoff-register.md) and (2) my three independent four-test stratified samples (Wave 1, Wave 2, Waves 3+4), run per SOP-087. Director approval and agent self-attestation are recorded but are explicitly not independence proofs. Method note (R-006, disclosed every time): my environment exposes file tools but no shell. Gate verdicts below are confirmed by reading the committed, generated register write-out and re-deriving the non-trivial verdicts against the reconciler source andinterface-contract-spec.md§3.1 — a trace, not a button-press. The COO/PMO must paste literalnodestdout (incl. a RED break-test) into the G3 checkpoint record to machine-attest the headline (open item OI-1).
0. Overall verdict (up front)
PASS WITH CONDITIONS.
The program clears the G3 / Program DoD bar. The single Critical finding from my
Waves-3+4 sample (C-1 — six registered, live cross-unit edges silently parse-dropped
behind a GREEN register) is remediated and confirmed cleared: all six edges now
reconcile matched/matched (fan-in) in the register, and the reconciler has been
hardened so the same parse-drop class can never recur silently (it now turns the gate
RED). The headline is now a complete-population GREEN — 403 edges, 0 defects, 3 pending,
and the 3 pending are legitimately platform-launch-gated (ADR-0006), not hidden defects.
This is PASS WITH CONDITIONS — not a clean PASS — because of residual non-blocking items the COO must carry into operation as open items, not gate blockers: a set of Medium staleness defects (registry-vs-prose drift, G-W2-1/2, M-1/F-1 class) that are real but do not touch any reconciled edge or the gate; the Archive batch for the Phase-2 changes is still pending (a gating step the Archivist owns before close, not a Q&R blocker); and the independence caveat SR-1 — my own unit’s Wave-3 artifacts were self-reviewed and require an independent External Review pass. None of these blocks program completion; each is a tracked carry-forward.
Aggregate executability across the three samples: 24/24 = 100% after the C-1 fix (see §5) — clears the ≥90% target (§10).
1. Criterion 1 — Handoff Register reconciles to 0 unmatched edges
PASS.
The committed, generated register (docs/handoff-register.md:13) reports:
GREEN — 84 files, 403 edges, 0 defects, 53 registered triggers, generated 2026-06-01.
This matches the tracker roll-up (§3.1/§3.2: “GREEN org-wide, 403 edges; 3 pending,
ADR-0006-gated”). I re-derived the non-trivial verdict classes against the reconciler and
§3.1 and found no edge that should report GAP/OVERLAP/UNREGISTERED reading as matched.
The 3 pending are legitimately platform-launch-gated (confirmed, not hidden defects).
They are register rows 252, 253, 255 — all Analytics Engineer → Design Research Director / UX Research Associate, trigger usage data ready
(SOP-049/051/053). I confirmed: (a) pending (counterparty not yet authored) appears on
exactly 3 edge rows (a 4th occurrence is the legend line :297, not an edge); (b) the
Analytics Engineer manual carries no ## Interface Contract block, so it is not an
authored owner and these edges correctly reconcile pending per §3.1 R3; and ©
substantively these are runtime telemetry feeds that cannot exist pre-launch (no
runtime data layer — ADR-0006; tracker R-005). They parse correctly and are visibly
pending — they are not in the C-1 parse-dropped class. The 3 pending are real
launch gates, not concealed gaps.
2. Criterion 2 — De-dup sweep complete; restated-canon defects → 0
PASS (with a stratified-coverage caveat — not a defect).
The Documentarian reports the de-dup sweep complete with 0 restated-canon defects found. My assurance basis is the three samples, which were stratified, not a census: across all three I read the non-redundant test on every sampled artifact and found 0 restated-canon passages — and several artifacts state the discipline explicitly (Finance charter “do not restate the field meanings”; Documentation charter operating-rule 2 “Copy-pasted policy/rule text is a defect, not coverage”; SOP-010/018/029 each linking canon rather than restating it). So the sweep result is consistent with everything I independently sampled.
Caveat (honest scoping, not a finding): “0 defects” rests on the Documentarian’s full-corpus sweep plus my stratified confirmation — it is not a Q&R full-census claim. A stratified sample that hits 0 across three waves is strong evidence the rate is at or near 0, but cannot prove a global 0. This is the normal limit of sampling (SOP-087), and it is adequate for sign-off. Conforms.
3. Criterion 3 — Cross-links resolve; glossary aligned; no contradictions with CLAUDE.md/ORG.md/canon
PASS (Documentarian Phase-2 pass), with carried Medium staleness items routed.
The Documentarian’s Phase-2 coherence pass (cross-link + glossary alignment) is reported
complete. My samples corroborate coherence on the reconciled surface: every sampled
Interface Contract interlocks, the trigger registry is the single controlled vocabulary,
and no sampled artifact contradicts CLAUDE.md/ORG.md/canon at the edge level.
The residual gap is the staleness class (Medium), and it is real but non-blocking. My samples found recurring registry-vs-prose drift: charters / operating-models / SOP-blockquotes that label now-registered, matched triggers as “(proposed)” / “pending” / “stubs until registered” (open control gaps G-W2-1, G-W2-2 from Wave 2; the M-1/F-1 class from Waves 3+4 — SOP-088/091/092/093/098 notes, coo-cluster operating-model). These are prose tables the reconciler does not parse, so they do not break the gate or contradict canon’s substance — but a reader cannot trust those specific tables until they are refreshed. Conforms for G3 (no canonical contradiction; gate unaffected) with the staleness items carried as OI-2 (route: CGEO / Growth / Creative / CPTO / COO own their respective prose; HR owns the Astro-5 def staleness G-W2-3). These extend a gap open since Wave 2 and should not be allowed to become standing.
4. Criterion 4 — KM reconciliation done; both KM indices reconcile
PASS.
The tracker reports KM index integrity GREEN 7/7 (§3.2) — km-index-check.mjs
enumerates every docs/knowledge/*.md and confirms each is present in the README index
(missing = []). I confirmed this class of gate in my Wave-1/2 samples by independently
reconciling the on-disk entry set against the README; the mechanism is sound and the
two-index discipline holds. KM reconciliation (durable "Next time"s promoted + linked) is
reported complete by the Documentarian. Conforms.
Carried housekeeping (not a gate blocker): control gap G-W1-1 (the
km-two-index-disciplineknowledge entry asserts a stale hard count “3 … all 3”) folds into the next SOP-024 curation pass. It does not move the KM gate (the index is GREEN either way) — carried as OI-2.
5. Criterion 5 — Q&R four-test sample passes (executability ≥90%) + N/A justifications pass (R11)
PASS — aggregate executability 24/24 = 100% post-fix; clears ≥90%.
This is the criterion the C-1 fix moves. Aggregating the three independent samples:
| Sample | Executable artifacts | Pass rate (as sampled) | Post-fix |
|---|---|---|---|
| Wave 1 | 4/4 job aids | 100% | 100% |
| Wave 2 | all sampled SOPs pass four tests | 100% | 100% |
| Waves 3+4 | 15/17 | 88% — driven entirely by C-1 (SOP-096, SOP-097) | 17/17 = 100% after C-1 fix |
| Aggregate | 24/24 | — | 100% |
The W3+4 88% was driven entirely by C-1, and C-1 is confirmed cleared:
- The six previously parse-dropped edges now reconcile cleanly in the register —
rows 207 (
health-score requirements issued→ D&A,matched (fan-in)), 211 (same, SOP-096), 212/213 (field-feedback routed→ Innovation / Design Research, matched (fan-in)), 216 (community signal raised→ CS,matched). The(**PROPOSED** — pending Operations)trailing prose is gone from SOP-095/096/097. - The two artifacts that failed executability because their declared edges were silently dropped (SOP-096, SOP-097) now pass — a fresh instance copying those Interface Contracts emits edges the gate sees and matches.
Recomputed aggregate clears the bar: with SOP-096/097 corrected, all 24 sampled executable artifacts pass → 100% ≥ 90%. The CGEO retention/field-feedback subsystem that was “invisible behind GREEN” is now visibly reconciled.
N/A justification sample (R11) — PASS. Across the three samples I sampled the
explicit N/A slots and confirmed each holds against the authoritative
artifact-matrix.md: Documentation/Finance/PA charter-manual collapses, DR playbook
N/A — accrue (a model R11 justification), the reserved SOP-046/047 numbers, and the
SOP-064/065/054/098 judgement-led job-aid N/As. No N/A slot in any sample was a
disguised empty/un-done slot.
6. Criterion 6 — Archive integrity 0 drift; both KM indices reconcile
PARTIAL — GREEN through Wave 2; Phase-2/Wave-3-4 archive batch is a pending gating step (Archivist-owned).
KM indices: GREEN 7/7 (criterion 4). Archive integrity: the tracker records
archive-manifest.sh verify = 0 drift / 0 missing through the Wave-2 registration
(§3.2). The Phase-2 changes (the C-1 edit, the de-dup/coherence pass, the Wave-3/4
artifacts) route to the Archivist before close and that batch is pending at the time
of this sign-off (tracker §3.2: “W3+4 batch pending”).
This is PARTIAL, not FAIL, and not a Q&R blocker. Archive integrity is the
Archivist’s lane (the archive-manifest.sh verify sweep — Q&R does not run it and
audits only whether it is run). The gating step is: the Archivist preserves the
Phase-2 batch and confirms verify = 0 drift / 0 missing before the COO signs G3. I
note this as a named pre-sign-off gating step (OI-3), owned by the Archivist, with the
PMO recording the literal result in the checkpoint record. Q&R’s sign-off is conditioned
on that sweep landing 0-drift; it does not block the substantive G3 verdict.
7. Criterion 7 — Sign-off rests on the scripted gate + Q&R sample, not self-attestation (R11)
PASS — and the control is now strengthened.
This sign-off rests on exactly the two R11-valid proofs: the scripted reconciliation gate (the register write-out, which parses actual declared edges and cannot be cleared by ticking a box) and the independent Q&R four-test samples. Director approval and agent self-attestation are recorded but explicitly excluded as independence proofs.
Strengthened control — parser-guard hardening (confirmed in source). The reconciler
now carries a malformed-edge guard (scripts/handoff-reconcile.mjs:240-301, 536-540):
an edge-shaped line whose trigger: opens a phrase delimiter but fails EDGE_RE (trailing
content after the closing delimiter, an unterminated delimiter) is no longer silently
skipped — it is pushed to malformed[], prepended to the defect list, and turns the gate
RED on its own (process.exit(1)). This directly closes the C-1 parse-drop class: the
exact failure mode that hid six live edges behind GREEN can now never recur silently — a
present-but-broken handoff turns the gate RED instead of vanishing from the population.
This converts a one-time C-1 remediation into a standing control, and it is the single
most important hardening in the convergence pass. I verified the guard logic in source; it
is load-bearing, not cosmetic.
Residual control-design observation (carried, not blocking): R-003 — the fan-in blind spot (a silently deleted fan-in producer still reads GREEN because the sink absorbs zero producers) — remains open. The malformed-edge guard closes the parse-drop sibling but not the deleted-producer sibling. The proposed compensating control (CC-1, a producer-presence CI assertion) is still owed by the Process Engineer. Carried as OI-4 in the risk register (R-003 stays Open). It does not affect the current GREEN (no fan-in producer is presently missing) — it is a defense-in-depth gap, not a live defect.
8. Independence caveat (SR-1) — Q&R’s own unit was self-reviewed
Disclosed, and External Review recommended.
My Waves-3+4 sample included Q&R’s own Wave-3 artifacts — the Q&R charter, SOP-085/086/087, the risk register, and the audit schedule. Those cells were sampled but explicitly marked self-review (not independent) — Q&R cannot independently assure its own anti-theater instrument (R11 applied to the second line itself). The other ten units in those waves had full independent treatment.
Recommendation (SR-1): the COO/Principal should commission External Review Partners (third line) for an independent pass on the Quality & Risk unit specifically — the charter, SOP-085/086/087, this sign-off, and the registers. This is the one part of the program assurance that a self-review cannot legitimately close. It is not a blocker for G3 (the Q&R artifacts conform on self-review and the rest of the corpus is independently assured), but it is a standing open item the COO carries into operation.
9. Per-criterion scorecard (master plan §9 “Phase 2 / Program DoD”)
| # | Program DoD criterion | Verdict | Evidence |
|---|---|---|---|
| 1 | Handoff Register reconciles to 0 unmatched edges | PASS | Register :13 — GREEN, 403 edges, 0 defects; 3 pending = rows 252/253/255, legitimately ADR-0006 launch-gated (telemetry, no ## Interface Contract on Analytics Engineer) |
| 2 | De-dup sweep complete; restated-canon → 0 | PASS | Documentarian sweep 0 found; 0 restated-canon in all 3 stratified samples (caveat: stratified, not census) |
| 3 | Cross-links resolve; glossary aligned; no canon contradiction | PASS | Documentarian Phase-2 pass; samples corroborate. Residual Medium staleness (G-W2-1/2, M-1/F-1) = prose-table drift, gate-unaffected → OI-2 |
| 4 | KM reconciliation done; both KM indices reconcile | PASS | km-index GREEN 7/7 (tracker §3.2); independently reconciled in W1/W2 samples |
| 5 | Q&R four-test sample passes, executability ≥90% + N/A sample passes |
PASS | Aggregate 24/24 = 100% post-C-1-fix (W3+4 88% was entirely C-1, now cleared); N/A sample holds across all 3 waves |
| 6 | Archive integrity 0 drift; both KM indices reconcile | PARTIAL | 0 drift through Wave 2; Phase-2/W3-4 archive batch pending (Archivist-owned gating step before COO signs) → OI-3 |
| 7 | Sign-off rests on scripted gate + Q&R sample, not self-attestation | PASS | This sign-off rests on the two R11 proofs; malformed-edge guard (reconciler :240-301) hardens the gate (parse-drop now RED) |
Six PASS, one PARTIAL (archive batch — a gating step, not a defect).
10. Overall verdict & residual open items for the COO
Verdict: PASS WITH CONDITIONS
The Documentation & Knowledge Program is substantively complete and real, not theater.
The headline integrity gate is a complete-population GREEN (403 edges, 0 defects), the one
Critical finding (C-1) is remediated and the failure class is permanently closed by a
hardened reconciler, the 3 pending edges are genuine launch gates, executability aggregates
to 100% across three independent samples, and N/A justifications hold. The C-1 remediation
even validates the program’s own thesis: the anti-theater bar caught a real “GREEN but
hollow” defect, and the fix made the gate strictly stronger.
It is PASS WITH CONDITIONS rather than clean PASS because of the residual items below. None is a blocker — each is an open item the COO carries into operation. I recommend the COO sign G3 → Principal once OI-3 (the archive sweep) lands 0-drift; the rest run as standing operational follow-ups.
Residual open items (carry into operation — NOT G3 blockers)
| ID | Open item | Owner | Why not a blocker |
|---|---|---|---|
| OI-1 | Machine-attest the gate. Operations/PMO paste literal node handoff-reconcile.mjs + km-index-check.mjs stdout (and one RED break-test) into the G3 checkpoint record. My gate verdicts are traces (no shell — R-006). |
Operations / PMO | Register write-out + source trace are strong; literal stdout converts trace → machine-attestation |
| OI-2 | Clear the staleness class. Refresh prose tables/blockquotes that label registered/matched triggers as “(proposed)/pending/stubs” (G-W2-1, G-W2-2, M-1/F-1 class) + Astro-5 def staleness (G-W2-3) + stale KM count (G-W1-1). | CGEO/Growth/Creative/CPTO/COO (prose); HR (defs); Documentarian (KM entry) | Prose the reconciler doesn’t parse; no gate/canon-substance impact. But open since Wave 2 — don’t let it become standing |
| OI-3 | Archive the Phase-2 batch. Archivist preserves the Phase-2/Wave-3-4 changes; confirm archive-manifest.sh verify = 0 drift / 0 missing before COO signs. |
Archivist (sweep) + PMO (record) | A gating step in the Archivist’s lane, not a Q&R blocker; Q&R audits whether it ran, not the checksums |
| OI-4 | Close R-003 (fan-in blind spot). Land the producer-presence CI complement (CC-1) so a silently deleted fan-in producer turns RED, matching the new parse-drop guard. | Process Engineer | Defense-in-depth gap; no live missing producer today; R-003 stays Open in the register |
| OI-5 (SR-1) | Independent review of the Q&R unit. Commission External Review Partners for a third-line pass on the Q&R charter, SOP-085/086/087, this sign-off, and the registers — Q&R self-reviewed its own Wave-3 artifacts. | COO / Principal (commission) | The rest of the corpus is independently assured; only Q&R’s own instrument lacks an independent pass |
11. Routing & preservation
- This sign-off is the Q&R G3 gate evidence; it routes to the COO (program owner) for the G3 sign decision → Principal.
- Open items route to named owners above (Operations/PMO · CGEO/Growth/Creative/CPTO/COO · HR · Documentarian · Archivist · Process Engineer · External Review Partners via COO).
- No new Critical/High is opened by this sign-off (C-1 is cleared); the risk register R-003/R-004/R-006 rows stay as carried (OI-1/OI-3/OI-4). No Principal escalation required beyond the routine G3 sign and the SR-1 commission.
- This report hands to the Archivist for preservation (
artifact approved) — and is itself part of the OI-3 Phase-2 archive batch.
Quality & Risk Director — program-level independent assurance (second line). Consolidates the Wave-1, Wave-2, and Waves-3+4 four-test samples per SOP-087. Q&R edited no artifact it reviewed; findings and open items are routed as proposals, not directives. The Q&R unit’s own artifacts are self-reviewed and recommended for External Review (SR-1).
