Program Completion Sign-Off — Org-Wide Self-Documentation & Knowledge Program (G3)
From: Chief Operating Officer (program owner —
docs/documentation-program/README.md§7 RACI “Program completion: COO approves → Principal”). To: the Principal, for acceptance. Gate: G3 (master plan §9 “Phase 2 / Program DoD”; tracker §1 checkpoint G3 “M5 → done”). Basis: the two R11-valid proofs only — (1) the scripted reconciliation gate (scripts/handoff-reconcile.mjs→docs/handoff-register.md) and (2) the Q&R independent four-test stratified samples consolidated in the assurance sign-off (reports/2026-06-01-program-assurance-signoff.md, PASS WITH CONDITIONS). Director approval and agent self-attestation are recorded but are explicitly not independence proofs. This memo does not re-derive the gate — it reads the Q&R sign-off, the tracker roll-up, and the cross-cutting log, and renders the COO’s completion call.
0. Verdict (up front)
Program COMPLETE (Phase 0 → Phase 2). Submitted for the Principal’s acceptance.
The Documentation & Knowledge Program clears the §9 Program DoD across all three axes — Coverage, Integrity (handoffs), and Coherence. The headline integrity gate is a complete-population GREEN, the one Critical finding from assurance is remediated and its failure class is permanently closed, and the residual items are non-blocking carry-forwards with named owners, not gate defects. I am signing G3 → Principal.
| Axis / gate | Status | Evidence |
|---|---|---|
| Coverage | 41 / 41 (100%) | tracker §3.1/§3.2 — every in-scope agent authored or N/A-justified; Q&R N/A sample held across all three waves |
| Handoff integrity | GREEN — 403 edges, 0 defects | docs/handoff-register.md:13; tracker §3.2; Q&R §1 PASS. 3 pending = rows 252/253/255, legitimately ADR-0006 launch-gated (runtime telemetry, no producer authorable pre-launch) |
| KM index integrity | GREEN 7/7 | km-index-check.mjs missing=[]; tracker §3.2; Q&R §4 PASS |
| Executability | 24 / 24 = 100% (≥90% bar) | Q&R §5 — aggregate across three independent samples, post-C-1-fix |
| Coherence | PASS | Documentarian de-dup + cross-link + glossary pass; 0 restated-canon in all three stratified samples (Q&R §2/§3) |
| Archive integrity | 0 drift (final batch landing now) | 0 drift through Wave 2; Phase-2/W3-4 batch is the Archivist-owned gating step (OI-3), being finalized concurrent with this sign-off |
This is a COMPLETE, conditioned sign-off — the conditions are the open items in §4, each routed to a named owner and carried into standing operation. None blocks completion.
1. What was built (the one-paragraph story)
We funded plumbing before volume. Wave 0 froze the Interface Contract block, the Handoff Register format, the trigger controlled-vocabulary, the reconciliation script, the KM templates and two-index check, and a worked exemplar — and cleaned roster hygiene — so that 41 agents would not produce 41 incompatible formats. Wave 1 stabilised the governance/foundational canon everyone cross-links to (Archives, Rules, HR, Documentation, Finance, Legal) before downstream waves wired to it. Wave 2 documented the CPTO cluster (the largest), with the CPTO enforcing internal consistency first. Waves 3 + 4 closed the COO and CGEO clusters. Phase 2 then converged all 41 agents into one coherent corpus reconciled against the Handoff Register until GREEN. The result is not a doc drive — it is the operating record the org now runs on: every agent owns its artifact set from unit charter down to job aid, every declared handoff names a counterparty who declares the matching edge, and a script — not a human eyeball — emits the single red/green number that says whether the seams hold.
The gate is real, not theater — and we have the receipts that prove it. Two “hollow-greens” were caught and killed during convergence, which is the strongest evidence the anti-theater bar works:
- The 47-seam identity gap. When all 41 agents were authored, extending the reconciler’s
IDENTITIEStable to name every role exposed 47 real cross-cluster seams that had been sitting hidden aspending(a GREEN that was green only because the reconciler could not see the edges). They were surfaced and resolved to matched (tracker §3.1). - The C-1 parse-drop. Q&R’s Waves-3+4 sample caught six registered, live cross-unit edges silently parse-dropped behind a GREEN register (the CGEO retention/field-feedback subsystem). This dropped executability to 88% on two artifacts (SOP-096/097) whose declared edges the gate could not see. The six edges were fixed to reconcile cleanly, and executability recomputed to 100%.
- Reconciler hardening (the standing control). The C-1 fix was not left as a one-time
patch. The reconciler now carries a malformed-edge guard (
scripts/handoff-reconcile.mjs:240-301,536-540): a present-but-broken edge no longer vanishes silently — it is pushed tomalformed[], prepended to the defect list, and turns the gate RED on its own (process.exit(1)). The exact failure mode that hid six live edges can never recur silently again. The anti-theater program caught a real “GREEN but hollow” defect and made the gate strictly stronger as a result — the thesis validated itself.
2. Cross-cutting dispositions (the COO cross-cutting log → close)
The Phase-0 cross-cutting High items (reports/phase0-reviews/coo-cross-cutting-log.md)
dispose as follows. Three of four are resolved; one carries a live external-compliance debt
owed by Engineering.
| # | Item | Disposition |
|---|---|---|
| H1 | Undisposed Golden Rule candidates | RESOLVED — Principal approved via GR-3 (GR-4 AI-workforce amendment + new GR-8 archive-integrity invariant); finalized in CLAUDE.md by Golden Rule. |
| H3 | Finance: budget approval + spend-materiality threshold unset | RESOLVED — Principal set the threshold (auto-approve one-time <$50 / recurring <$20-mo; at/above → Principal) and chose track-first budgeting; SOP-026/027 now executable. |
| H4 | AI-usage ledger three-unit seam (Finance ⨉ D&A ⨉ AI Systems) | RESOLVED (Wave 2) — D&A (SOP-062) + AI Systems (SOP-065) + AI Eng (SOP-067) produce usage data ready; Finance SOP-028 fan-in sink consumes; reconciles matched. |
| H2 | Live Slack ToS compliance gap (ADR-0017) | OPEN — code fix owed by Engineering. The app_uninstalled deletion failure is silently swallowed (intranet/src/pages/api/slack/events.ts:303-305); migrations + fallback job unbuilt. Security posture is documented (SOP-083), the deletion cron is disabled until fixed, and the risk is tracked as R-001 in the Q&R risk register. This is a real external-compliance exposure that the documentation program documented and contained but does not itself close — the remediation is an Engineering code deliverable, not a doc artifact. It is the one cross-cutting item with live external exposure and stays Open under Engineering (code) + Security (posture). |
The honest line on H2: the program’s job was to make the org self-knowing, and it did — the gap is now visible, posture’d, and contained rather than silent. But “documented” is not “fixed.” H2 carries into operation as a live code debt, and I keep it on the COO surface until Engineering lands the fix and Security confirms the posture closes.
3. Standing operation (the corpus is now live)
Program completion is not program end — it is the transition to standing operation. The corpus the org runs on is now live and maintained through the mechanisms the program built:
- Unit charters + SOPs are the system of record for how the work runs; each is owned by
its unit/Lead and reviewed on the canon cadence (
docs/canon.mdRACI; on-change for charters, quarterly for SOP/job aid). - Knowledge (
docs/knowledge/) is the append-only inbox of how we got smarter; the Documentarian curates and promotes durable learnings into the corpus, the two-index check keeps it reconciled, and supersession (not expiry) governs the record. - The integrity gates run in CI —
handoff-reconcile.mjs(now with the malformed-edge guard),km-index-check.mjs, andarchive-manifest.sh verify— so the seams, the KM index, and the archive stay GREEN continuously rather than by audit. Until AR-003 wires the cadence automation, the PMO runs the manual sweep at each checkpoint (CR-1/CR-2 on the COO risk log). - The quarterly review cadence keeps freshness in band; the staleness class (OI-2 below) is the first thing that cadence clears.
4. Open items carried into operation (Q&R OI-1…OI-5)
These are the conditions in “PASS WITH CONDITIONS.” Each is routed to a named owner; none blocks G3. I track them on the COO surface until closed.
| ID | Open item | Owner | COO note |
|---|---|---|---|
| OI-1 | Machine-attest the gate. Paste literal node handoff-reconcile.mjs + km-index-check.mjs stdout (and one RED break-test) into the G3 checkpoint record, converting Q&R’s source-trace verdict (no-shell, R-006) into a machine-attestation. |
Operations / PMO | The register write-out + source trace are strong evidence; literal stdout is the last mile from trace → button-press. Required for the checkpoint record, not for the substantive verdict. |
| OI-2 | Clear the prose-staleness class. Refresh prose tables/blockquotes that still label now-registered/matched triggers as “(proposed)/pending/stubs” (G-W2-1/2, M-1/F-1 class), the Astro-5 def staleness (G-W2-3), and the stale KM count entry (G-W1-1). | Unit Directors (CGEO/Growth/Creative/CPTO/COO own their prose; HR owns the defs; Documentarian owns the KM entry) | Prose the reconciler does not parse — no gate or canon-substance impact. But open since Wave 2; the first quarterly cadence clears it so it does not become standing. |
| OI-3 | Archive the Phase-2 batch to 0-drift. Preserve the Phase-2 / Wave-3-4 changes and confirm archive-manifest.sh verify = 0 drift / 0 missing. |
Archivist (sweep) + PMO (record) | The one gating step, in the Archivist’s lane — being finalized concurrent with this sign-off. My G3 sign is conditioned on this batch landing 0-drift; it does not move the substantive verdict (Q&R audits whether it ran, not the checksums). |
| OI-4 | Close R-003 (fan-in blind spot). Land the producer-presence CI complement (CC-1) so a silently deleted fan-in producer turns the gate RED, matching the new parse-drop guard. | Process Engineer | Defense-in-depth: the malformed-edge guard closes the parse-drop sibling; CC-1 closes the deleted-producer sibling. No live missing producer today — R-003 stays Open in the risk register, not a live defect. |
| OI-5 (SR-1) | External Review of the Quality & Risk unit. Commission External Review Partners (third line) for an independent pass on the Q&R charter, SOP-085/086/087, the assurance sign-off itself, and the registers. | Principal (commission, via COO) | Q&R cannot self-close this — it self-reviewed its own Wave-3 artifacts, and a second line cannot independently assure its own anti-theater instrument. The rest of the corpus is independently assured; this is the single part that a self-review legitimately cannot close. This one is a Principal decision, surfaced here for the acceptance call. |
5. The completion call
The program is substantively complete and real, not theater. Coverage is 41/41; the Handoff Register is a complete-population GREEN (403 edges, 0 defects); the KM index is GREEN 7/7; executability aggregates to 100% across three independent Q&R samples; the one Critical finding is remediated and its failure class permanently closed; and the residual items are named, owned carry-forwards rather than concealed gaps. The two hollow-greens we caught and killed are the proof the gate is a gate.
I sign G3 — program COMPLETE (Phase 0 → Phase 2) — and submit it to the Principal for acceptance, with the five open items above carried into standing operation and OI-5 (External Review of Q&R) flagged as the one item requiring a Principal decision.
6. Routing & preservation
- This memo is the COO G3 completion sign-off; it routes to the Principal (via the Personal Assistant if not direct) for acceptance.
- Open items route to their named owners (§4). OI-5 awaits the Principal’s commission decision.
- Hands to the Archivist for preservation (
artifact approved), and is itself part of the OI-3 Phase-2 archive batch.
Chief Operating Officer — program owner. Built on the Q&R independent assurance sign-off (second line) and the PMO delivery tracker. The COO authored no artifact it signs off on beyond this memo and the program plan; assurance and reconciliation are independent of this sign-off. Hand to the Archivist; do not declare the record closed until OI-3 lands 0-drift.
