Docs carry last-reviewed: + owner in a comment; a periodic staleness sweep
(operating-cadence) flags overdue ones.
The org-wide rollout of these obligations — the per-agent documentation set
(charter → manual → SOP → job aid), the handoff-integrity gate, and the knowledge
lifecycle (capture → curate → preserve → index → retrieve) — is governed by the
Documentation Program (docs/documentation-program/, Operations/COO). That
program is the authority for the rollout’s phasing, RACI extensions, and KM
supersession governance; this section remains the standing RACI it reuses.
